Le dossier pharmaceutique : du DP patient au DP rupture, un formidable outil de santé publique créé par les pharmaciens
Résumé
Le pharmacien dispose depuis quelques années d’un nouvel outil, le dossier pharmaceutique (DP), qui lui permet d’assurer une analyse pharmaceutique plus exhaustive. Il enregistre dans le DP les médicaments délivrés avec ou sans ordonnance médicale. La création d’un dossier pharmaceutique requiert l’accord ou le consentement du patient. Les données de santé à caractère personnel qui y sont contenues sont non modifiables par le dispensateur consultant. Seul le patient a un droit permanent d’accès et de rectification. Le dossier, qui peut être clôturé à la demande du patient, l’est automatiquement s’il n’a fait l’objet d’aucun accès pendant 5 ans.

Abstract
Since several years, the pharmacist has got a new tool: the pharmaceutical folder which makes him able of more precise pharmaceutical analysis. In this folder, he enters delivered drugs, with or without medical prescription. The creation of the folder requires patient consent. The personal health datas can not be modified by the consultant. The patient is the only one to have a permanent right of access and modification. The folder, which can be closed on patient demand, is automatically closed if there is no activity during five years.
